Rail firm fined £1m over train passenger's death

Great Western Railway fined £1m for breaching health and safety laws leading to passenger's death. Bethan Roper, aged 28, died in 2018 after her head struck a tree branch when leaning out of a droplight window. ORR found GWR aware of the risk but failed to implement safety measures, leading to the tragic incident. Measures have since been introduced in the rail industry to prevent such accidents.
